March 2023 Workshop: Interventional Treatments for Mood and Anxiety Disorders

Interventional treatments show increasing promise in the treatment of psychiatric disorders, particularly treatment resistant depression (TRD) and obsessive-compulsive disorder (OCD). Development of tools and techniques including deep brain stimulation (DBS), transcranial magnetic stimulation (TMS), and electroconvulsive therapy (ECT), as well as novel medications may yield additional options for patients who fail to respond to standard pharmaceutical and psychotherapeutic treatments.  This presentation reviews the motivation for and use of these treatments. We provide a brief description of the illnesses followed by discussion of the circuit models thought to underlie these disorders. These circuits provide targets for intervention. Using understanding of these basic mechanisms, we discuss mechanisms and techniques for novel interventional treatments. The discussion focuses on novel pharmacological tools, particularly ketamine/esketamine and brexanolone, as well as neuromodulatory treatments: DBS, TMS, and ECT.

Dr. Kyle Lapidus is the founder and medical director of Affective Care, which provides psychiatric treatment for a wide range of patients, particularly focused on optimizing function for the highest achieving people and managing the most difficult and treatment-resistant patients with mood and anxiety disorders. After receiving his BA magna cum laude from Harvard University, Dr. Lapidus earned his MD and PhD at Albert Einstein College of Medicine; he completed his residency in psychiatry at Mount Sinai Medical Center's Icahn School of Medicine, where he served as chief resident for research in the Physician-Scientist track. He is board certified by the American Board of Psychiatry and Neurology.
